Fee: $150 per 60-minute session

Effective March 2020: In-network provider for the following insurance companies:

Anthem Blue Cross/Blue Shield (Blue Preferred Network, Blue Access Network, Blue Traditional, Workers’Comp, Indiana Exchanges HMO/POS, Anthem HealthSync)

Community Health Alliance (Aetna - Aetna Select, Elect Choice EPO, Managed Choice plan MC, Open Access Aetna Select, Open Access Elect Choice EPO, Open Access Managed Choice plan MC, Choice POS II, Open Choice PPO Plan, Anetna Signature Administrators ASA, NAP + Aetna PPO, Aetna Student Plans), Meritain, Passport to Health and Trinity

CO-PAY due at time of each appointment in cash, check or credit card.

Private pay clients who do not wish to use insurance will be expected to provide payment due at time of each appointment. Effective January 1, 2022, a ruling went into effect called the “No Surprises Act”, which requires mental health practitioners to provider a “Good Faith Estimate” GTE about out-of-network care to any client who is uninsured or who is insured but does not plan to use their insurance benefits to pay for mental health care. I will provide you a written estimate based on information known at the time of the estimate and review this in our first session.

Individuals out of network who wish to file insurance can be seen but verifying what benefits your insurance company provides for therapy to an out of network provider will be critical. Please be aware that a number of insurance companies provide no benefits for out of network providers.

Clients should call their insurer to verify out-of-network insurance benefits and where to submit a reimbursement claim.
Be sure to ask the following questions:

• Do I have mental health benefits?
• What is my deductible and has it been met?
• How many sessions a year does my health insurance cover?
• Where would I send receipts for mental health services to be reimbursed?
• Is pre-approval required by my primary care physician before I can seek mental health services?
